  • Insurance Notice
    We do not accept private insurance, Medicare, or Medicaid.

 

  • Payment Terms
    This is a self-pay, pay-per-visit service.
    Payment is collected at the beginning of the virtual visit.

 

  • Additional Procedures
    Any procedure or service needed during your visit will be discussed beforehand to ensure transparency and consent.

 

  • After-Hours & Holiday Surcharge
    A 50% surcharge applies to consultations scheduled:
    • Between 7:00 PM and 7:00 AM
    • On weekends and public holidays

3. Please complete all forms thoroughly, review and sign the documents authorizing Dr. Ayala to treat you. You'll then see a calendar with available dates. For appointments outside regular hours, please email Dr. Ayala
